I've had Tusker's tusks trimmed every two years (which makes twice now), and the vet sedates him, uses a Gilateen Cutter, and only trims the tusks to about half an inch from the gum. Tusker is normally very docile and well-mannered, but when his tusks get long he gets very aggressive with our dogs - not us, but just the dogs. Once the tusks are trimmed, he's just fine and everyone gets along.

It's a very pronounced personality change, and it's really been very easy to keep the tusks trimmed since our vet comes out to the house.
