If the tusks get too long, they can grow back into the cheek. A friend of mine rescued a neutered male that was about 4 years old and his had done that. She trimmed the tusks and had to treat the holes that were left for infection. I have a four year old that usually breaks them off accidentally when they get long. Right now he has one that is about 3 inches long and is growing back toward his cheek. It has about 1 inch to go. I'm going to have to try and trim it. Wish me luck :)
